Position Summary
Moda Health Coaches develop and implement behavioral-based coaching programs and health care navigation support, including condition-specific disease management (health coaching), for Moda members covered under the City of Portland health plan participating in the Healthy Foundations program.  This position collaborates with internal and external healthcare services teams, vendor point solutions, and community-based organizations to ensure continuity and coordination of care. This is a FT WFH role. 

 

Pay Range
$58,747.10 - $73,433.88 annually (depending on experience)
*This role may be classified as hourly (non-exempt) depending on the applicant's location. Actual pay is based on qualifications. Applicants who do not exceed the minimum qualifications will only be eligible for the low end of the pay range.

Required Skills, Experience & Education:

  1. Current Oregon license as a Clinical Social Worker (LCSW), Professional Counselor (LPC) or other mental/behavioral health accreditation.
  2. Bachelor’s degree, preferably in healthcare-related field.
  3. Ability to work collaboratively and independently.
  4. Excellent communication, interpersonal, and presentation skills.
  5. Strong cultural competence and understanding of the importance of using Diversity, Equity, and Inclusion (DEI) concepts to guide and inform work.
  6. Thorough knowledge and understanding of health behavior change theory.
  7. Experience with Microsoft Office products.
  8. Ability to learn proprietary databases and maintain confidentiality.

 

Preferred Education & Experience:

  1. Master’s degree in a related field.
  2. Minimum of one year of experience in field related to health coaching.
  3. Experience in health insurance or similar industry.


Primary Functions :

  1. Works one-on-one with members managing, or at-risk for developing chronic health conditions by providing health coaching via multiple communication channels, including phone, email, mail, virtual and in-person, to support disease prevention and management.
  2. Using evidence-based guidelines, provides education and support to help members set and achieve health-related SMART goals and increase their capacity as a self-manager of their health.
  3. Assesses member health through initial consultations and evaluations, including exploring visions, motivation, readiness to change, and health history. Routinely documents member progress, changes in health behaviors, and/or gaps closures in EHR and customized database systems.
  4. Partners with internal and external healthcare services teams to achieve triple-aim goals of improving healthcare experience, outcomes, and reducing avoidable cost.
  5. Contributes to the selection, development, and review of health education materials, and other group and member-facing wellness communications.
  6. Occasionally provides health education presentations to group employees on topics related to healthy lifestyle and condition management.
  7. Maintains knowledge of vendor health management programs and refers members to programs, as needed.
  8. Performs other job-related duties, as assigned.
